The detail most guides gloss over

The 25% cap is monthly, not daily

The indirect-care and clinical-administration cap (Q312 + Q313 combined) is averaged over each calendar month, per OMA guidance, not measured day by day. A heavy charting day is fine, as long as the month balances out.

Money left on the table

Trimming legitimate Q312 hours on a heavy admin day because a daily readout looks alarming, even though the monthly ratio had plenty of room.

Quiet drift toward the cap

Stacking several charting-heavy days without a monthly view, and only discovering the problem at reconciliation. The ratio caps aren't system-enforced until April 2027, so today's overpayment can become tomorrow's clawback.

It's monthly, not daily. Per OMA guidance, the indirect-care and clinical-administration ratio caps are averaged over each calendar month, not measured day by day, so a heavy charting day is fine as long as the month balances out.
Q310 is direct patient care at $80/hour, Q311 is telephone care from outside the office at $68/hour, Q312 is indirect care (chart review, labs, referrals) at $80/hour, and Q313 is clinical administration at $80/hour. All four are billed in 15-minute units.
